Release of Information

All releases or transmittals of medical records, information and preparation of copies of these documents, i.e., personal copies civilian providers, insurance requests, legal requests and VA/disability requests, are provided through the Medical Records Department. Due to the large volume of requests, customers requesting duplication of their medical records should submit their requests in writing no less than 30 business days in advance.
