TRICARE Prime Remote

TRICARE Prime Remote is available to active duty service members and their eligible family members who live and work in designated remote locations in the United States (50 miles or an hour drive time from a military treatment facility [MTF]).

  • Click here to see if you may qualify for TRICARE Prime Remote based on your home and work ZIP codes.

Once you've decided that TRICARE Prime is right for you, you'll select a primary care manager and then you have three options to enroll.

Step 1: Select a Primary Care Manager
If there are network primary care managers in your area, you will select a primary care manager on your enrollment form:

If not, you can select any TRICARE-authorized provider as your primary care manager. Contact your regional contractor to ensure the provider is certified.

Step 2: Enroll Online, Via the Mail or In Person

1. Online: Log on to the Beneficiary Web Enrollment Web site and follow the instructions to enroll online.

Note: If you are required to pay enrollment fees, you will be unable to complete your enrollment online at this time. However, the enrollment form is available for you to download.

2. Via the Mail: Download the TRICARE Prime Enrollment and PCM Change Form for your region and mail it to your regional contractor. The address is found on Page 3 of the enrollment form.

3. In Person: Go to a TRICARE Service Center to complete and submit the form. Find one near you:

After your regional contractor processes your application, you will receive a welcome letter identifying your primary care manager (PCM), if assigned, along with an enrollment card for each family member that has been enrolled.  Your TRICARE Prime Remote start date will be printed on your enrollment card.  Additionally, you'll receive a copy of the TRICARE Prime and TRICARE Prime Remote Handbook.

Please keep a copy of your enrollment application until you have received your enrollment card.  If you must seek care before receiving your card, contact your regional contractor to verify your TRICARE Prime Remote start date and PCM assignment.

Family Member Coverage
Family members must reside with the sponsors to enroll. If you have family members who live in a separate location from the TRICARE Prime Remote-enrolled sponsors, they may enroll in TRICARE Prime if its offered where they live or they can be covered by TRICARE Standard and Extra.

Note: Family members of activated Guard/Reserve members may enroll if they reside designated remote location at the time of activation.

Determination of Eligibility Request
Only active duty service members may request a determination of eligibility requirements for TRICARE Prime Remote enrollment. You or your Unit Commander may submit an online request form if you believe that one or more of the exceptions to the eligibility criteria are met.

  • Unit is in one region, but service member lives in another region
  • Unit and service member are located in one region, but closest primary care provider is in another region
  • Geographic barriers or other unique situations exist (e.g. the drive time to the nearest MTF exceeds one hour)

If you think you qualify, submit a TPR Determination of Eligibility Enrollment Request Form online.
