Applications and Forms
Applications may be mailed in and Fees can be paid using Pay.gov. Failure to follow these guidelines will delay the processing of your
application. You may mail your application to the nearest REC.
Please make sure your application is complete to avoid any
delays.
Application Status: you
can check the status of your current Application for Merchant
Mariner Credential (MMC) or Certificate of Registry
(Form CG-719B).
A physical examination performed by a licensed medical provider is necessary for determining a mariner's fitness for duty. The examination and form must be completed within 12 months of the application date. Please remember to fill out the form using ALL CAPS.
Acceptable Evidence for Identification - According to CFR Title 46 part 10.203(d):
"Although an MMD and an MMC serve as certificates of identification, a TWIC must be retained by the mariner to whom it was issued and, while valid, serves as the mariner's primary identification document. The TWIC must be produced to verify identity when required by an authorized official as identified in 33 CFR 101.515(d)."
Mariners who desire to send correspondence or applications to a Regional
Examination Center, should use the correct address (available here)
to
ensure that a return address is shown on the envelope. Incorrectly addressed
mail will result in delays in processing. If sending original or certified
copies of documents, it is recommended that you use certified mail or another
mail service that provides tracking capabilities. Originals will be returned
to you after verification.
