Determination of Eligibility Request

Only active duty service members may request a determination of eligibility requirements for TRICARE Prime Remote enrollment. The service member or Unit Commander may submit an online request form if you believe that one or more of the exceptions to the eligibility criteria are met.

• Unit is in one region, but service member lives in another region
• Unit and service member are located in one region, but closest primary care provider is in another region
• Geographic barriers or other unique situations exits (e.g. the drive time to the nearest military treatment facility exceeds one hour)

Call your regional contractor and ask for assistance before completing the Determination of Eligibility Request online:
