Use the information below to request a Defense Occupational and Environmental Health Readiness System – Environmental Health (DOEHRS-EH) database account.

For first time users, the DOEHRS demo site is recommended for familiarization, can be found at https://doehrs-ih-demo.csd.disa.mil

         *Program Office: DoD Deployment Surveillance 
         *Requested Permission(s) 
                   - Standard User Roles: Environmental Health User Role 
                   - Equipment Administrator: None 
                   - System Administration Roles: None 
         Mobile Synchronization Account: Do not check 
         Oracle Discoverer Account: Check “Oracle Discoverer Viewer Access” 
         * Justification for Access to DOEHRS: 
                   - “User needs access to DOEHRS to support OEHS missions.”
