Eligibility
The Active Duty Dental Program is available to the following beneficiaries:
Active Duty Service Members
You will qualify for the Active Duty Dental Program in one of the following scenarios:
- You are referred by a military dental treatment facility (DTF) to a civilian dentist for any reason. All referred care will be coordinated by the military DTF with United Concordia. You may not self-refer to a civilian dentist if you live or work within 50 miles of a military DTF.
- You live and work in a remote location, more than 50 miles from a military DTF. You must select a dentist from United Concordia's dental provider network.
Note: If you are covered by the Transitional Assistance Management Program (TAMP), you are not eligible for the Active Duty Dental Program.
National Guard/Reserve Members
You will qualify for the Active Duty Dental Program when you are activated for more than 30 consecutive days (under Federal orders), and you are:
- Referred for dental care from a military dental treatment facility, or
- You live and work in a remote location while on active duty.
You may also qualify in the following scenarios:
- Up to 180 days before you go on active duty if you receive delayed-effective-date orders
- When covered by the Transitional Assistance Management Program after serving on active duty for more than 30 days in support of a contingency operation
- When you are no longer on active duty or covered by TAMP and the dental care needed is related to a line of duty injury (official documentation is required)
