Remote Care
You'll receive all dental care through the TRICARE Active Duty Dental Program if you live and work:
- in a designated remote location in the U.S. (more than 50 miles from a military dental treatment facility),
- Puerto Rico,
- Guam,
- the U.S. Virgin Islands,
- American Samoa, or
- the Northern Mariana Islands
You may coordinate your routine covered dental services (e.g. examinations, cleanings, fillings) as long as the dental treatment is less than $750 per procedure or appointment or the cumulative total is less than $1,500 for treatment plans completed within a consecutive 12-month period.
To make a routine dental appointment, you must first get an Appointment Control Number (ACN) from United Concordia by completing an Appointment Request Form. You'll have the option to make the appointment or allow United Concordia to make the appointment for you.
For any specialty dental care (e.g. crowns, bridges, dentures, periodontal treatment), you must first have authorization from United Concordia. To receive authorization, have your civilian dentist complete an Authorization Request Form and indicate the requested services.
