Monthly Premiums


Monthly premium rates are based on the sponsor's military status (active duty, Selected Reserve or Individual Ready Reserve [IRR]) and type of enrollment:

  • Sponsor only
  • Single enrollment (one family member; does not include sponsor)
  • Family enrollment (more than one family member; does not include sponsor)
  • Sponsor and family

The rates listed below are for May 1, 2012 - January 31, 2013. After that, premium rates will be adjusted annually every February.

Active Duty Family Members 
  • Single: $10.30 per month
  • Family: $30.89 per month 
Selected Reserve,
IRR (Mobilization Only) & Family Members
  • Sponsor only: $10.30 per month
  • Single: $25.74 per month
  • Family: $77.22 per month
  • Sponsor and family: $87.52
IRR (Non Mobilization) & Family Members
  • Sponsor only: $25.74 per month
  • Single: $25.74 per month
  • Family: $77.22 per month
  • Sponsor and family: $102.96

The Single and Family rates do not include the sponsor

Initial Premium Payment
Monthly premium amounts are based on your sponsor's military status. An initial (one month) premium payment must be submitted when you enroll:

  • If enrolling online, pay your initial premium payment with a credit card
  • If enrolling through the mail, pay with a check or money order. Please include the sponsor's Social Security number or DoD Benefits Number in the memo area.

Ongoing Recurring Payments
If the sponsor has a military payroll account, and if sufficient funds are available, the government will collect the sponsor's share of the premium through a Uniformed Services Finance Center and transmit to MetLife. This payment method is only available for sponsors of active duty family members, and National Guard and Reserve sponsor premiums.

If you cannot set up a military payroll allotment, you have the option to set up a recurring payment via electronic funds transfer (EFT) from your checking or savings account or by credit/debit card.

  • If enrolling online, you can set up the recurring payment (payroll allotment, EFT, or credit/debit card) at the same time.
  • If enrolling through the mail, complete Section IV of the TRICARE Dental Program Enrollment/Change Authorization document.
    • Include a voided check to establish the EFT from your checking or savings account.
    • If using a credit or debit card for the recurring payment, include the type of card, card number, expiration date and security code. Then be sure to sign the form.

Failure to pay your monthly premiums will result in a termination of your coverage and you will not be able to re-enroll for one year (12-months).

Monthly Premium Transition Update
As a result of system changes taking place during the transition from United Concordia to MetLife, some Reserve and National Guard members paying monthly premiums by payroll deduction may need to pay MetLife directly. We are working on a solution and apologize for any inconvenience. Beneficiaries affected by this issue will not be disenrolled, but if you receive a paper bill from MetLife, please make your payment by check, electronic funds transfer or credit card until payroll deduction options become available.
