TRICARE Young Adult


TRICARE Young Adult is a premium-based health care plan available for purchase by qualified dependents. When you purchase TRICARE Young Adult, you will have access to medical and pharmacy benefits, but dental coverage is excluded.

  • Follow the steps below to purchase the plan.

1. Download the TRICARE Young Adult Application
Download the TRICARE Young Adult Application for your region.

If you want to select the US Family Health Plan as your Prime option, download the TRICARE Young Adult Application for your US Family Health Plan provider:

You may live in a different region than your sponsor. If you're not sure which region you live in, click here to see which states are in each region.

2. Select Your Coverage Option: Prime or Standard
Follow the following guidelines to complete #1 on the form to select your coverage option:

  • Mark "TRICARE Prime" to use the Prime Option.
  • Mark "TRICARE Standard" to use the Standard Option
  • Mark "TRICARE Overseas Prime" to use the Prime Option in all overseas areas (remote or non-remote). Only command-sponsored children of active duty service members may mark this box.
  • Mark "TRICARE Reserve Select" if your sponsor is enrolled in TRICARE Reserve Select. When you check this box, you will be using the Standard Option.
  • Mark "TRICARE Retired Reserve" if your sponsor is enrolled in TRICARE Retired Reserve. When you check this box, you will be using the Standard Option.
  • Mark "TRICARE Prime Remote for Active Duty Family Members" if your sponsor is enrolled in TRICARE Prime Remote. Only children of active duty service members may mark this box.

3. Mail or Fax the Completed Application
Mail or fax the completed form and initial three-month premium-payment to your regional contractor or US Family Health Plan Provider, whichever applies. Mailing addresses and fax numbers are found on the form. Make checks or money orders for the initial premium payable to your regional contractor or US Family Health Plan Provider.

After your regional contractor processes your application, you will receive a welcome letter along with an enrollment card. Your TRICARE Young Adult start date will be printed on your enrollment card. It is different depending on which coverage option you choose. Please keep a copy of your application until you have received your enrollment card.  If you must seek care before receiving your card, contact your regional contractor to verify your TRICARE Young Adult start date.

Need a new uniformed services ID card?

You can enroll in TRICARE Young Adult without a valid uniformed service ID card. Once your application is processed, and you've received your welcome letter and enrollment card, please visit any ID card office to get a new ID card. You should present your ID card and enrollment card when you go to the doctor.
