Forms Instructions
(03/06/2008)As a service to our public and the trade community, U.S. Customs and Border Protection (CBP) has made available electronic forms that can be completed online, saved and printed. The forms were created in Adobe Reader Extension software requiring the free Adobe Reader version 7.0.9 to reach full functionality. All Federal accessibility requirements established under Section 508 of the Disabilities Act have been met.

Forms Trouble ShootingThe full Adobe form will only work with 7.0.9 version or higher. If a form does not open up in pdf for you, please make sure you have the current Adobe Reader. You may download a free Adobe Reader from the Adobe website.
