This application allows users to view county-level maps of heart disease and stroke by racial/ethnic group, along with maps of social environmental conditions and health services for the entire United States or for a chosen state or territory. - Data Trends & Maps
The DHDSP Data Trends & Maps website is a national repository of resources that document the public health burden of cardiovascular diseases and their risk factors at the national, state, and local level to inform planning, implementation, and evaluation of prevention measures. The DHDSP website development project is part of a chronic disease-wide integrated surveillance data model that will facilitate sharing of risk factor data (smoking, diet, physical activity, etc.) across the CDC divisions working in chronic disease. - National Heart Disease and Stroke Maps
Static maps of heart disease and stroke mortality and hospitalizations by county can be viewed and downloaded. - Social Determinants of Health Maps
Maps of selected social determinants of health provide information to be used in tandem with other data sources to tailor heart disease and stroke prevention programs and policies to the needs of local populations. - Heart Disease and Stroke Printed Atlases
Published atlases are available to download or order a hard copy, for example, 2010 Atlas of Heart Disease Hospitalizations Among Medicare Beneficiaries.
