Special Diabetes Program for Indians (SDPI) Community-Directed Grant Program Continuation Application Information for FY 2013

APPLICATION FOR CONTINUATION FUNDING

The application package and instructions for FY2013 are available. Carefully read all instructions before starting to prepare your application.

  • Application Package:
    • Electronic access and submission of non-competitive applications will no longer be accepted through Grants.gov, and must be submitted through GrantSolutions.gov:
      1. Log on to http://www.grantsolutions.gov Exit Disclaimer – You Are Leaving www.ihs.gov
      2. Click on “Apply for Non Competing Award”, this link will be available under “My Grants List”.
      3. Enter all the application information in different forms.
      4. After completing all the forms, click on “Verify Submission.”
      5. Click on “Final Submission.” Click on “OK” upon the prompt “Are you sure you want to submit application?”

INSTRUCTIONS & APPLICATION TEMPLATES

  • Project Narrative Template and Other Documents - Use these documents to prepare your application and ensure that you have included all required documents in your application package. GrantSolutions.gov update! In order to upload files in GrantSolutions.gov, all documents must be saved under a file name with no special characters (ex. %_$!).
    • SDPI FY 2013 SDPI Continuation Application Checklist [PDF - 49KB] – Appendix 2 from SDPI instructions.
    • Key Contacts Form [PDF - 40KB] – This form is required as part of your application. Complete and save the form electronically, then attach it to your application package using the "Other Attachments Form."
    • Project Narrative Templates – For FY 2013, the Project Narrative will consist of two parts: 1) the main Project Narrative template and 2) individualized Best Practice templates. Carefully read the instructions and review the entire template before starting to prepare your program’s Project Narrative. In order to upload files in GrantSolutions.gov, save and rename all templates on your computer without any special characters (ex. %*$!&).
  • Other Activities Template [Word – 164KB] – Use this template for any other objectives and activities that do not fit
    in your chosen Best Practice(s). Note: using the Other Activities template does not count as a Best Practice.

DEADLINES FOR CONTINUATION APPLICATIONS

  • Budget Cycle 1 (budget period end date 9/30/2012): June 1, 2012
  • Budget Cycle 2 (budget period end date 12/31/2012): August 31, 2012
  • Budget Cycle 3 (budget period end date 3/31/2013): November 30, 2012
  • Budget Cycle 4 (budget period end date 5/31/2013): February 1, 2013

Note: These dates are set as guidelines per grants policy and are subject to change.

APPLICATION FORMS FOR APPLICANTS WITH SUB-GRANTEES OR SUB-CONTRACTORS down arrow up arrow

These forms are provided only for planning purposes or for use by applicants with sub-grantees or sub-contractors. The grants.gov application package includes the official versions of these forms that must be completed by all applicants.

ENVIRONMENTAL REVIEW AND DOCUMENTATION FORM (NEPA) down arrow up arrow

The Environmental Review and Documentation Form is required for any grant project that includes building, remodeling, excavation, purchasing modular structures, change of land use, and related project activities. If you are required to use this form, see the IHS Environmental Review Manual for detailed information. This manual contains the form along with instructions for its use.
