Project Description: This research has two main aims:

1. Evaluate whether high-intensity quality improvement reduces the risk of contrast-induced acute kidney injury (CI-AKI).
2. Evaluate whether CI-AKI is associated with the development of CKD, progression of CKD, development of end-stage renal disease, repeat revascularization, and long-term mortality.

Career Goals: Dr. Brown is an Assistant Professor of Health Policy and Clinical Practice at the Dartmouth Institute for Health Policy and Clinical Practice, Dartmouth Medical School. He received his PhD in Health Services Research from Dartmouth followed by an American Heart Association post-doctoral fellowship in cardiovascular epidemiology at the Dartmouth Medical School. The grantee was then awarded a NRSA fellowship in Health Services Research at the Dartmouth Institute for Health Policy and Clinical Practice. Dr. Brown intends to become an independent health services researcher with a focus on patient safety and subsequently, become an expert on the interaction between chronic kidney disease and cardiovascular disease.

Progress to Date: For Aim #1, the grantee has completed the 1-year baseline for the pre-intervention phase. Focus groups were conducted at 10 of the participating medical centers. For Aim #2, 7287 patients have been enrolled.

Future Plans: The next phase for Aim #1 will be a 3-year intervention period of continuous high-intensity quality improvement. Enrollment for Aim #1 will continue and analyses on the VA PCI cohort will be conducted.
