REACH Communities
CDC's REACH program currently funds 50 grantee partners through REACH U.S. (40) and REACH CORE (10). Grantees focus on the following population racial/ethnic groups: African American/Black, American Indian/Alaska Native, Asian, Native Hawaiian/Other Pacific Islander, or Hispanic/Latino.
REACH funded partners focus on eliminating health disparities in one or more of the following health priority area(s): breast and cervical cancer; cardiovascular disease; diabetes mellitus; adult/older adult immunization, hepatitis B, and/or tuberculosis; asthma; and infant mortality.
