Authority: 5 U.S. C. 301, Departmental Regulations, 10 U.,S.C. 133, Under Secretary of Defense for Acquisition, Technology and Logistics DoD Instruction 7050.01, Defense Hotline Program; DoD Instruction 5200.22, Reporting of Security and Criminal Violations; and DoD Instruction 5505.6, Investigations of Allegations Against Senior Officials of the Department of Defense

Purpose: Records the hotline complaint given by an individual in connection with an incident, accident or suspected violation under investigation, regardless of the individual's relationship to the investigation.

Routine Uses: The Blanket Routine Uses Published by DLA and set forth at the beginning of DLA's compilation of system of records notice.

Disclosure: Voluntary
