Page Help for Advanced Search

The Advanced Search page allows you to search both the National Coverage and Local Coverage databases using a variety of criteria such as document ID, keyword, diagnosis/procedure code, and date.

Document ID: Enter the Document ID in the text box to search. You must specify the entire ID including any dashes or letters. Please note that MEDCAC Meetings, Technology Assessments, and Medicare Coverage Documents do not have a Document ID.

Document Type(s): Using the radio button options, you can search for National Coverage Documents, Local Coverage Documents, or both National and Local Coverage Documents. The radio button options also have additional Document Type check box criteria.

Geographic Area / Contractor (Local Coverage only): If your search includes Local Coverage Documents, you must select a Geographic Area and/or Contractor information. If you are entering Contractor information, you may choose to enter a Contractor Type(s) or a Contractor(s) by Name.

Keyword(s): Enter a keyword, or multiple keywords, in the text box to search. You can specify the scope of the search (“Title” or “Entire Document”) as well as the functionality of the search operation [“Exact Phrase,” “All Words (and),” or “Any Words (or)].” If you are using the Keyword(s) text box, you must enter a minimum of two (2) characters.

CPT/HCPCS (Local Coverage only): Enter a specific CPT/HCPCS code or a code range in the text box corresponding to your radio button selection. If you are entering a code range, you must enter the start and end code.

ICD-9 (Local Coverage only): Enter a specific ICD-9 code or a code range in the text box corresponding to your radio button selection. If you are entering a code range, you must enter the start and end code. Click on the ‘Code Lookup’ link to search for a specific code using a keyword search.

Date Criteria: This search function enables you to target documents that were in effect during a certain date. The default radio button option is “Currently in effect,” but you can also specify a specific date or a range of dates.

NCA/CAL Status (National Coverage only): If your Document Type search includes either NCA or CAL documents, you can select an NCA/CAL status option in the drop-down box.

Benefit Category (National Coverage only): If your Document Type search includes NCDs documents, you can select one or more Benefit Category options in the multi-select box.

'Back to Top' Link: The 'Back to Top' link anchors to the top of the page.

Only 'Final Policies' (under LCDs) is selected by default so that only finalized LCDs display. If you would like to view results for Draft LCDs, please select 'Draft Policies'.
