What is DHAP?
The Deployment Health Assessment Program (DHAP) allows you take proactive steps in protecting your health and well-being, as well as ensuring readiness. There are three deployment health assessments (DHAs) taken during your deployment cycle. 
The Pre is taken prior to deployment, the Post is taken during redeployment and the Reassessment (PDHRA) is completed 90-180 days after redeployment.
 
The DHAs have three simple steps: (1) Resilience Training (Comprehensive Soldier Fitness), (2) a specific electronic questionnaire (DD Form) completed through the Medical Protection System (MEDPROS), and (3) a confidential, one-on-one conversation with a health care provider.  Each step, along with your honest participation given in a timely manner, will ensure accurate snapshots of your health.

Who takes the DHAs?
All Soldiers and DA Civilians who deploy OCONUS for greater than 30 days to locations with non-fixed U.S. medical treatment facilities (MTFs).
 
Where do you take the DHAs?
Specific guidance will be provided by your Commander. Some will participate at an on-site event.  Others maybe instructed to complete the electronic questionnaire (DD Form) online and then phone the call center. Again, your Commander will provide that guidance.
 
Why should you take the DHAs?
• Your health impacts your future, your family and your job, the DHAs exists so that you can take proactive steps to protect your health.
• DHAs provide you snapshots of your behavioral and physical health following during the deployment cycle, which may be vital information should injuries or health concerns arise in the future.
• You get to take care of any conditions which may effect your personal readiness and deployment status.
• It allows you to examine behaviors or feelings that may be indicators of a deeper issue, and receive appropriate care. These steps can reduce stress and strain for yourself and your loved ones.
