FOIA Forms

The following forms are used in the FOIA Program as prescribed in DLA FOIA Program, 32 CFR 300.4 (f), or DoD Freedom of Information Act Program, DoD 5400.7-R. The forms are used by DLA FOIA Offices to facilitate the processing of FOIA requests.

DL1835	Freedom of Information Act Request Cover Sheet	Oct 2000	This form is used within DLA to identify FOIA packages to ensure priority action.
DL1917	FOIA Request for Documents, Action Office Search / Review Information Sheet	Aug 2011	This form is used within DLA by the FOIA Offices to assist the action offices in tracking their search and review activities.

DD2086	Record of Freedom of Information (FOI) Processing Cost	Jan 2003	This form is used throughout DoD to record costs associated with the processing of a Freedom of Information request.
DD2086-1	Record of Freedom of Information (FOI) Processing Cost for Technical Data	Jul 1997	This form is used throughout DoD to record costs associated with the processing of a Freedom of Information request for technical data.
DD2086-2	Freedom of Information (FOI) Consultation and Request Summary	Jul 2008	This form is used throughout DoD to record the disposition and costs associated with processing a request, consultation, or appeal.
