Abstract
Older patients and those with complex needs are at risk of medication error when they transfer between care settings. A Royal Pharmaceutical Society project is improving the information accompanying a transferring patient. Nurses are adopting the ideas, including use of a 'green bag' containing patients' medicines and medicines information.
