Helpful Hint:If you are not using Windows 95 or higher, you may need to re-boot and only open the Internet browser, return to the LI-APP page and click on the PDF file to load it into the adobe reader for printing.

Important Notice: If you are skillful you may be able to fill out the application in the reader or adobe exchange software; the latter allows you to save the file, the reader does not permit saving so you must enter the required information and print the file before exiting the program because the data will not be saved in the file.

2. HTML

Helpful Hint: The HTML LI application may be filled-out online and printed.

Important Notice:
The HTML LI cannot be saved online or in your system. It MUST be printed to retain the information you inputted. Closing or saving the document will result in the permanent loss of your data.
