Exceptional Family Member Program (EFMP)

     The EFMP is a mandatory enrollment program that coordinates with military and civilian agencies to provide community support and comprehensive medical, educational, housing, and personnel considerations to Families with special needs.

What is an Exceptional Family Member (EFM)?

     An Exceptional Family Member is defined as an authorized Family Member who requires special medical, educational, and/or mental health services based on a diagnosed physical, intellectual, and/or mental health/behavioral disability.

     An authorized Family Member may be a spouse, child, stepchild, adopted child, foster child, or a Family Member parent. Disabilities may range from mild to severe.  Special needs to consider for identification of EFMP enrollment include, but are not limited to, medical requirements, developmental or educational services, mental health treatment, housing considerations, adaptive equipment, or assistive technology devices and services.

How the program works

     The EFMP ensures continuity in the Exceptional Family Member’s care and education as the Family transfers from assignment to assignment, commonly referred to as a Permanent Change of Station (PCS). The program combines the efforts of the personnel and medical commands, the Family Support Center, and the educational system.  The term Exceptional Family Member Program, or EFMP, actually refers to two functions in the military services:

The personnel function

• Identifies Family Members with special medical, educational, and/or psychological needs, documents the services they require, and considers the needs when making personnel
assignments.
• Involves the personnel and medical commands and the DoD educational system overseas.
• Is standard across all branches of the services.

 The Family support function

• Assists the Service Member with coordinating military and community services for Exceptional Family Members.
• Involves Family Support Center staff.
• Differs from service to service. (EFMP is available as a Family support function in the Army and Marine Corps only.)

Getting enrolled

     It’s important to start the enrollment process as soon as your Family Member is identified as eligible for the EFMP.

     To enroll, re-enroll, or dis-enroll, you may contact the Case Coordinator by calling 270-956-0614. 

          You may download a DD Form 2792 Medical Summary and/or DD Form 2792-1 Educational Summary (and if applicable, bring the child’s IFSP or IEP) for completion depending on the enrollment eligibility criteria identified. 

     BACH EFMP hours are (with the exception of holidays, days of no scheduled activity DONSA):

Monday, Tuesday, Wednesday, Thursday, Friday
8:00 AM to 11:30 AM and 1:00 PM to 4:00 PM

Developmental-Behavioral Pediatric Clinic

     If your child has a developmental, learning, and/or behavioral concern, a referral to see our Developmental-Behavioral Pediatrician may be necessary. Parents who feel their child may need   an evaluation must first discuss their concerns with the child’s Primary Care Provider, at which time a referral to EFMP may be made. All referrals are reviewed by the Developmental-Behavioral pediatrician and a determination is made on whether the concerns can be addressed in our clinic or if the child may benefit from being seen elsewhere.

     Appointments are scheduled for Monday, Wednesday, and Thursdays from 8:00 AM to 3:30 PM. Appointments are scheduled on a referral basis only; not all referrals to EFMP automatically result in an appointment to see the Developmental-Behavioral Pediatrician.

     Useful information to bring with you, if an appointment is scheduled, is a list of current medications, prior records from testing done in the school or by early intervention, and the necessary forms or questionnaires provided by our office specifically for the appointment.

Overseas Screening

     Per AR 608-75a, all Family Members of Active Duty Service Members are to be screened through EFMP for any special medical or educational needs.  The screening will take place with a Health Technician. EFMP alerts assignment managers at personnel agencies to consider the documented medical and education needs of the Family Members in the assignment process.  EFMP enrollment is mandatory if the Family Member is found to meet criteria during the screening process. 

     A DA Form 5888, with #1-8 completed, is required prior to the screening.  This is obtained at the Family Travel office following notification of a pending overseas assignment.  A DA Form 7246 will be completed at the time of the screening to determine eligibility for enrollment for all Family Members applying to travel overseas with the Service Member. 

     If Family Members meet criteria for medical enrollment, a DD Form 2792 will be given to take to the designated physician for completion.  If educational enrollment is necessary, a DD Form 2792-1 will be completed by student’s early intervention agency or school accordingly and a copy of their Individualized Family Service Plan (IFSP) or Individualized Education Plan (IEP) must be provided.
