Form I-9, Employment Eligibility Verification
Purpose: This form is used to verify your citizenship and eligibility to work in the United States.
Instructions: Complete Section 1. Print one copy and sign the form. On your first day of work, bring a passport or driver’s license and your Social Security card as required proof of citizenship.

Form OF-306, Declaration of Federal Employment
Purpose: This form is used to determine your acceptability for federal employment.
Instructions: Read and complete the form. Print one copy and sign the form. Note: A false statement on any part of this form may be grounds for not hiring you or for firing you after you begin work. You may also be punished by fine or imprisonment.

Purpose: This form is used to ensure that your prior service is verified. Your length of service will affect leave and retirement benefits.

Form SF-256, Self Identification of Disability
Purpose: This form is used to collect statistical information for agency reports on hiring, placement and advancement of handicapped individuals.
Instructions: Complete the requested personal information at the top of the form and indicate the appropriate code. Print one copy and sign the form.

Form SF-181, Ethnicity and Race Identification
Purpose: This form is used to collect statistical information on the composition of the workforce. Completion of this form is optional. However, if you do not complete it, USCIS will attempt to identify your race and national origin visually.

Form AD-349, Employee Address
Purpose: This form is used to designate your official mailing/residence address so that you can receive your bi-weekly Earnings & Leave Statements and other important mailings.
Instructions: Read and complete only blocks 1 through 7. Print one copy of the form. Sign and date the form at steps 13 and 14.

Form W-4, Employee’s Withholding Allowance Certificate
Purpose: This form is required by the Internal Revenue Service (IRS) and is used to determine the correct amount of federal income tax to be withheld from your bi-weekly earnings based on the number of exemptions that you claim.
Instructions: Step 1: Use the personal allowances worksheet to help you determine the number of withholding allowances that you are entitled to claim.
Step 2: Fill out the Form W4 and print one copy.

State Withholding Exemption Certificates
Purpose: This form is required by the IRS and is used to determine the correct amount of state income tax to be withheld from your bi-weekly earnings based on the number of exemptions that you claim and the state where you live. This site links to all state withholding certificates.
Instructions: Step 1: Determine the number of withholding allowances that you are entitled to claim.
Step 2: Fill out the withholding exemption certificate and print one copy.
Step 3: Sign and date.

Form F-1199A, Direct Deposit Sign-Up
Purpose: This form is used to identify the financial institution where your bi-weekly earnings will be forwarded via electronic funds transfer. If you wish to have voluntary automatic payroll deductions for any other voluntary purpose (such as savings accounts and, car payments), ask about it at orientation.
Instructions: Read and complete the form, print two and sign each copy. Please have the routing number for your financial institution available. To obtain the routing number, contact your financial institution.

Form SF-61, Appointment Affidavits
Purpose: This form is used on your first day of work when you take the Oath of Office. This form is NOT to be completed until you are sworn in.
Instructions: Print one copy. Bring this blank form with you on your first day. DO NOT COMPLETE. After you are sworn in, you will sign and date the affidavits.

Form D-4A, Certificate of Non-residence in the District of Columbia
This form is required for persons working at USCIS Headquarters whose residence is not within in the District of Columbia.
Purpose: This form is required by the IRS and is used to notify the District of Columbia (D.C.) to not deduct taxes from your wages because you do not reside in D.C.
Instructions: Download and complete the form. Bring the form with you on the first day of your new-employee orientation.
Who Submits: Employees who work at USCIS Headquarters in Washington, D.C., and who do not reside within the District of Columbia.
