EA-09-249 Ginna	NOV (White)	11/12/2009	On November 12, 2009, the NRC issued a Notice of Violation to Constellation Energy for a violation of 10 CFR 50, Appendix B, Criterion XVI, “Corrective Action,” at the R.E. Ginna Nuclear Power Plant. The violation, which is associated with a White Significance Determination Process finding, involved the failure to identify the cause of a significant condition adverse to quality. Specifically, after identifying corrosion on the turbine-driven auxiliary feedwater pump governor control valve stem on April 11, 2005, the licensee did not take adequate measures to identify the cause or prevent recurrence. This led to additional corrosion and binding of the governor control valve, and resulted in failure of the turbine-driven auxiliary feedwater pump on July 2, 2009.
EA-09-045 Ginna	NOV (White)	06/08/2009	On June 8, 2009, the NRC issued a Notice of Violation to Constellation Energy for a violation of Technical Specification 5.4.1.a, “Procedures” at the R.E. Ginna Nuclear Power Plant.  The violation, which is associated with a White Significance Determination Process finding, involved the failure to implement a Technical Specification-required procedure.  Specifically, in March 2008, the licensee did not implement steps for cleaning and lubricating the turbine-driven auxiliary feedwater pump’s governor linkage assembly, as required.  Failure to conduct this preventative maintenance led to the turbine-driven auxiliary feedwater pump being declared inoperable when the governor linkage became stuck, preventing the pump from obtaining the required discharge pressure and flow during surveillance testing in December 2008.
EA-08-075 Ginna	NOV (SL III)	04/07/2008	On April 7, 2008, a Notice of Violation was issued for a Severity Level III violation, due to changes the licensee made to its NRC approved emergency plan; specifically they made changes to the emergency action levels (EALs), between 1996 and 2001, without first obtaining Commission approval.  This decreased the effectiveness of the emergency plan.   The violation cited the licensee for not following 10 CFR 50.54(q), which requires, in part, that a licensee authorized to possess and operate a nuclear power reactor shall follow and maintain in effect emergency plans which meet the standards in 10 CFR 50.47(b) and the requirements in appendix E of this part.
EA-04-003 Ginna	NOV (SL III)	03/30/2004	On March 30, 2004, a Notice of Violation was issued for a Severity Level III violation involving the willful failure to follow procedures by a manager when he manipulated two valves during a plant cooldown without authorization as required by procedure.
EA-02-141 Ginna	NOV (White)	10/04/2002	On October 4, 2002, a Notice of Violation was issued for a violation associated with a White SDP finding involving the alert and notification system (ANS). The violation cited failures involving the ANS that could have affected the ability to provide early notification to the public.
