The state of New York

The Western New York Beacon Community plans to expand the western New York network, close gaps in service, and improve health outcomes for patients with diabetes. Specifically the goals of the program are to:

  • Reduce emergency department visits, hospitalizations, and readmissions for influenza, pneumonia, and congestive heart failure in patients with diabetes
  • Increase percentage of diabetic patients with blood sugar, cholesterol, and blood pressure control
  • Improve smoking cessation and flu immunization for diabetic patients
  • Reduce health disparities, especially in both urban and rural underserved areas, using health information technology (including clinical decision support systems, patient portals, and telemonitoring) to facilitate patient monitoring and treatment
  • Expand access to health information among patients
