John Graham, PhD, PMP, and Cynthia Oakes from the Southern Piedmont Beacon Community also called the "Southern Piedmont Community Care Plan Beacon Community", discuss how their Beacon Community will help increase use of health information technology, including health information exchange among providers and increase patient access to health records to improve coordination of care, encourage patient involvement in their own medical care, and improve health outcomes while controlling cost.
