Steps for Processing Premium-Class Accommodations:
Step 1:
Traveler initiates request for other than economy/coach class accommodations, providing:
Medical Documentation Requirement Format form completed and certified by competent medical authority and dated within six months of travel
Traveler's Request for Premium-Class Travel form that includes: travel fare amount in U.S. dollars for coach-class and other than economy/coach class accommodations (nearest dollar), ticket issuing location (name and location of CTO), and
Copy of travel orders must be submitted as soon as they are completed by DLA. The Medical Documentation Requirement Format form and the Traveler's Request for Premium-Class Travel form should be submitted first.
Step 2
Submit to DoDEA via e-mail or facsimile
Step 3
Medical consultant retained by DoDEA reviews certification and provides recommendation
Step 4
Review by DoDEA Office of General Counsel
Step 5
Review by DoD Office of General Counsel
Step 6
Acting DoDEA Director coordinates
Step 7
ASD (R&FM) reviews
Step 8
PDUSD (P&R) approves/disapproves
Step 9
If approved, DoDEA OCA notifies DLA for amendment of travel orders
Step 10
With amended travel orders traveler request ticket from CTO
Step 11
CTO issues ticket
Emergency Circumstances:
Will process without orders steps 2 through 6;
however, orders are required for steps 7 and 8.
