Recipient Responsibilities: Reporting Requirements

You will be ineligible for continuation of scholarship support and your scholarship payments will be discontinued if you fail to submit these forms or documentation as required.

Recipient's Initial Program Progress Report

You must submit a Recipient's Initial Program Progress Report (IHS-856-8)* (PDF - 131KB) form to your IHS Scholarship Program analyst within 30 days from the beginning of each academic term. This form must be signed by your school advisor or the registrar's office verifying that you are enrolled in a full-time or part-time course load for the current academic term.

Transcripts

You must submit an official transcript within 30 days from the end of each academic period — semester, trimester, quarter or summer session. Official transcripts must have the institutional seal and/or the signature of the registrar. Copies of official transcripts or electronically produced grade reports will not be accepted. Do not remove official transcripts from your institution's original mailing envelope. Transcripts that appear to have been handled and/or altered in any way will not be accepted. Benefits will be suspended if official transcripts or other approved documentation are not submitted within 30 days.

You must have your official transcripts sent directly to your IHS Scholarship Program analyst once they become available. If an official transcript will not be available within 30 days, you must submit a copy of your official grade report or documentation indicating the grades you received in each class. This report or documentation must be signed by each instructor and your advisor.

Notification of Academic Problem

You must submit a Notification of Academic Problem (IHS-856-9) (PDF - 131KB) form to your IHS Scholarship Program analyst if you experience an academic problem at any time during the academic term.

Under no circumstances should you withdraw from a course on your own without consulting your IHS Scholarship Program analyst. If you do so, the IHS Scholarship Program will automatically place you on academic probation and you will jeopardize your scholarship funding and benefits.

Examples of academic problems where you should use this form include:

  • Reduction in credit hours
  • Cancelled course
  • Academic probation
  • Withdrawal from school
  • Dismissal from school
