ANNOTATION: The efficacy of a community-based public health model, Transitional Health Care Coordination, “that makes use of periods of incarceration to identify chronic health and mental health needs of inmates…[in order to] provide continuity of care and to facilitate the containment of communicable disease through community health care providers” is examined.
