EFMP enrollment is mandatory and required immediately upon identification of a special need.   The required forms for enrollment are:

• DD Form 2792, Exceptional Family Member Medical Summary for medical issues only,  and;  
• DD Form 2792-1, Exceptional Family Member Special Education/Early Intervention Summary for educational issues.

To determine if you are already enrolled in EFMP, follow the Service specific links below:.

• Air Force: Air Force service members can find out by contacting their installation's Special Needs Coordinator.
• Navy: Navy service members can find out their enrollment category by visiting the Navy's EFM Program Status Verification web page.
• Marine Corps: Marine Corps service members can find out their enrollment category by contacting their assigned Regional or Installation EFMP Coordinator.

Enrollment in EFMP does not harm one's military career. The aim of the Exceptional Family Member Program is to consider a family member's special need during the assignment coordination process. Sponsors are still deployable according to the needs of the Service.
