Regional Investigations
The Office of Aviation Safety investigates almost 1700 aircraft accidents each year, most of which are general aviation accidents investigated by regional staff. While these generally do not have the public visibility of major air carrier accident investigations, these investigations can still involve safety issues important to the traveling public.

September 2012
- 9/30 - At 1:40 PM MDT, Safford, AZ, Experimental weight shift-control light sport kit, crashed in remote mountainous terrain on the San Carlos Indian Reservation.
- 9/30 - At 10:30 AM CDT, Eastland, TX. Agusta Spa. A-109E helicopter operated by CareFlite, impacted terrain about 6 miles south of the Eastland Municipal Airport.
- 9/29 - At 5:15 PM EDT, Fredericksburg, VA. A Cessna 150M crashed after departing Shannon Airport.
- 9/22 - At 3:30 PM, CDT, Roanoke, TX. Piper PA-28R-180 impacted terrain on initial takeoff.
- 9/22 - At 12:55 AM EDT, Tampa, FL. Cessna 172 crashed in cow pasture.
- 9/16 - At 3:21 PM CDT, Brownsboro, AL. YAK 52 VFR local flight crashed.
- 9/15 - At 12:01 PM EDT, West Windsor Township, NJ. Aerospatiale AS-355F1, VFR from Princeton, NJ to Atlantic City, NJ crashed.
- 9/15 - At 7 AM MST, Strawberry, AZ. Wreckage of Piper PA-28, on instructional flight missing since 3 PM yesterday, located in mountainous terrain.
- 9/15 - At 12:30 AM CDT, Springfield, MO. Cirrus SR-22, IFR flight plan Lee's Summit, MO to Springfield crashed while on instrument approach for landing.
- 9/15 - At 12:15 AM MDT, Cuchara, CO. Mooney M20C on VFR flight plan from Pueblo, CO to Reno, NV, missing since 9/13/12, was located on Teddy's Peak in the Sangre de Cristo Mountains.
- 9/13 - At 7:41 PM EDT, Cumming, GA. Experimental RV-7 crashed.
- 9/10 - At 4:00 PM CDT, Houston, TX. A Robinson R22 crashed in an industrial area.
- 9/10 - At 10:20 AM CDT, Lansing, IL. Airboss Gyrocopter impacted terrain shortly after takeoff.
- 9/9 - At 12:03 PM CDT, Kansasville, KS. Beech A36 impacted terrain.
- 9/8 - At 6:42 PM CDT, Kingsland, TX. Piper J3C-65 impacted terrain after takeoff.
- 9/6 - At 1:46 PM CDT, Winnsboro, LA. E-AB Lancair IV impacted terrain after takeoff.
- 9/5 - At 11 AM ADT, Anchorage, AK. A DHC-8-100 ERA Alaska #877 while in cruise at 12K' in moderate icing experienced temporary loss of control. Crew able to regain control at 7K' and landed without further incident.
- 9/3 - At 1 PM MDT, Murtaugh, ID, Skykits USA Corp, light sport aircraft, crashed in mountainous terrain.
- 9/1 - At 1:30 PM CDT, Davenport, IA. L-39 Experimental, Warbird Jet, part of a 3 aircraft formation demonstration at the Quad Cities Airport Air Show departed controlled flight and crashed in a field.
- 9/1 - At 11:02 AM EDT, Falmouth, MA. Cirrus SR22 crashed shortly after takeoff.
- 9/1 - At 9 AM EDT, Brighton, MI. Aeronca 7AC Champ, while flying in the traffic pattern at the Brighton Airport, departed controlled flight and crashed into a swamp behind a residential area.
- 9/1 - At 9 AM EDT, McVeytown, PA. Experimental, amateur-built Safari helicopter crashed in a soybean field during a solo instructional flight.
