System Change Request Form

SYSTEM CHANGE REQUEST (SCR) FORM

This SCR form is routed to the MC4 Helpdesk. If the SCR is deemed valid it will be entered into the MC4 Helpdesk system and you will receive a tracking number. A valid request is one that describes the issue in complete detail. The request must also contain complete contact information for the submitter. Also, the issue must not be a duplicate of a previously reported change request.

MC4 will vet the SCR through the MC4 Technical Review Board (TRB) and then send it to the MHS Helpdesk. At this point, the SCR will receive an MHS tracking number. To track the progress of your SCR, please keep all tracking numbers on hand and request updates from support@MC4helpdesk.com.

It is important to note that most system changes can take more than five years to incorporate.
