How can Soldiers obtain copies of their EMRs from deployment?

MC4 is unable to release medical records or information on any Service member. It is recommended that Soldiers contact the nearest military treatment facility’s medical records department to ensure HIPAA procedures and protection of personal health information is adhered.

If electronic records are not found, Soldiers may contact the U.S. Army Patient Administration Systems and Biostatistics Activity (PASBA) for paper records. PASBA reviews, performs data quality checks, and enters the medical information into a number of databases and then forwards the records to the National Personnel Records Center (NPRC) in St. Louis, Mo. For deployed medical record requests, problems or assistance contact the PASBA.
