How to respond or get help with your form

If you live in or manage group housing such as a dorm, nursing home, barracks, etc., please click here for special instructions.

Fill out the form and mail it back in the postage-paid return envelope.
If you have trouble answering a question, call us and we will help you.

If you don't mail back your form or if you leave questions blank, we may call you.

We may also call if you have 6 or more people at your address, since the mail-in form only has room for 5.

In-Person Interview

If we don't hear from you by mail or phone, a Census Bureau field representative may visit you in person to help you complete the American Community Survey.
