To request the services of a neutral from the DoD Roster, organizations submit the following documents to the Center for ADR by regular mail or facsimile:  Request for Mediation and Agreement to Mediate in Good Faith.  Because the majority of requests are for mediation services, a request form for mediation was created.   Requestors who are interested in other ADR processes can adapt the mediation request form to their needs or submit a request containing the basic information in whatever format they choose. 

 The employees of the Center for ADR are keenly aware that information provided in requests for neutral services is often personal, proprietary or otherwise sensitive and such information is rigorously protected.
