Application Instructions

Complete the Application Form below, then click [Review and Submit Form] when finished. Click on the ? to the right of each field for input instructions. Note that fields with a gray background and a trailing asterick (*) are required, and that only plain text may be used (no Microsoft Word or Adobe PDF formatting allowed.)

On the Review Submission page, you'll have the chance to review your input, and select [Submit] to submit your application, or [Go Back] to make changes, before reviewing it again.

You can also have your submission emailed to you by completing the "Notification Email" field before you submit.

Community/Project Name *	?	Enter the name of your spotlight community or name you have given to your project or partnership.
Project Location (City, State) *	?	Enter the general location where your community activities take place.
Federal Agencies involved: (Please note that there is a minimum of two federal agencies required to participate. Please spell out each agency.) *	?	In order for your application to be considered, there must be at least 2 federal agencies involved.
Applicant Contact Information *	?	Enter the applicant contact information, including name, title, agency, email and phone number.
Spotlight Community Federal Members (Name, Agency/Organization) *	?	Enter the names of the Spotlight Community federal members. Please include name, and agency or organization.
Contact information for the senior manager who has reviewed the application (Name, Title, Agency, Email, Phone) *	?	Enter the contact information for the senior manager who has reviewed the application, including name, title, agency, email and phone number.
If your project or initiative involves any non-federal stakeholders, please list those participant organizations	?	If your project or initiative involves any non-federal stakeholders, please list the name of each participant organization.

Any Federal facility or installation involved in a GreenGov Spotlight Community application will be subject to an environmental compliance status review. This process will include consideration of information in the Environmental Protection Agency's Online Tracking Information System (OTIS), which is available online at: http://www.epa-otis.gov/otis/ (click this link for help in using OTIS), as well as other available compliance and enforcement information.
