PHA's are obtained by appointment between 0700-1100 Monday through Friday. Thursday afternoons the medical clinic is closed for training. Please call 510-437-3658.
TO SUCCESSFULLY COMPLETE A PERIODIC HEALTH ASSESSMENT:
Complete the HEALTH RISK ASSESSMENT by logging on to the Fleet Health Risk Assessment (HRA) website: https://nmcpeh-web2.med.navy.mil/pls/newhra/dsc6pop.NEHCha_Login
- Enter the UIC/OPFAC: 31090
- Once the HRA is completed online, click the "FINSHED" button
- This will generate a Health Risk Appraisal Participant Report, PRINT THIS OUT PLEASE.
- Bring a copy of the report and your Health Record with you.
TO SUCCESSFULLY COMPLETE A PERIODIC HEALTH ASSESSMENT: (Retirement, Separation, RELAD, Aviator physical, Officer candidate, OMSEP) There are different preliminaries for each type of physical. However all items below are required:
- Fasting lab work (12 hr NO food and NO drink: WATER OK!). Lab is walk-in 0630-1100 M-F
- Current dental exam
- The documents needed are: DD2807 and DD2808

Mon/Tue/Wed & Fri 0700-1100 & 1230-1400. Thu 0700-1100 only. Thursday afternoon closed for training.

If you are requesting your lab results please schedule an appointment to review the results with the provider that ordered the test. The technician is not authorized to give you results directly.

The clinic contracts for routine visual exams. Appointments are available for retirees and dependents on a space available basis (after active duty needs are met).
Examinations for contact lenses are not provided.
For active duty and retiree members only, glasses may be ordered in the lab department after your eye exam. It takes 2-6 weeks for glasses to come in. If you are an active duty member you will be be notified via e-mail and if you are a retiree you will receive a phone call when the glasses arrive.
