Distance Education Students Attending Second Residence Courses
Please fax a copy of your APFT (DA Form 705, Army Physical Fitness Test Score Card) to include your body fat statement (DA Form 5500 or DA Form 5501) if necessary, and or profile (DA Form 3349) to the USAWC Registrar.
