Purpose & Scope
The purpose of this inspection-in-depth was to determine the adequacy of the State's construction inspection procedures for concrete bridge deck construction. The inspection also reviewed various project aspects to determine if (STA) provided proper contract administration and maintained the required quality control relative to the bridge construction operations.

BATCH PLANT OPERATIONS
- Materials Handling and Storage
- Are the stockpiles separated to prevent intermingling? What is the time between stockpiling and using? Are stockpiles covered? Are they free draining?
- If the plant was inspected by District:

DECK PLACEMENT OPERATIONS
- Forming
- Describe method of forming deck.
- Are deck forms adequately cleaned out?
- Are forms oiled or wetted prior to pour?
- Reinforcing Steel
- Is reinforcing steel clean?
- Are rebars adequately tied?
- Was reinf. checked for clearance, and how?
- Was horizontal & vertical spacing checked?
- Is steel adequately supported? Describe.
- Are appropriate bars epoxy-coated? Is coating damaged?
- Concrete Placement
- How many cubic yards is the pour? ______C.Y.
- Is the contractor positioned and prepared @ the beginning of the pour?
- Are screed rails anchored properly as approved by the Engineer?
- Was a dry run made with the screed?
- What is the evaporation rate? Is it recorded? Is there noticeable wind during placement?
- What was the temperature when concrete placed? Ambient: Deck:
- Does the contractor have enough manpower on pour?
- Describe how the concrete is being placed:
- Placed by chute/bucket/pump/conveyor
- Adequate vibration? How many vibrators?
- Are the concrete delivery tickets properly filled out:
- What is the delivery time to the job?
- Is added water noted? Are at least 40 additional revolutions required to mix the added water?
- Are mixer drum revolutions recorded & less than 250?
- Are test results & cylinder numbers noted?
- Pour should be continuous, if stopped for one hour did they place a header?
- Describe the finishing process.
- Are they adding water to the surface?
- Are they over working the surface?
- Is the surface closed by the screed finishing or by hand work?
- Describe the curing process.

- Is the field testing done properly?

- If pumps or conveyors are used, were correlation tests performed & results incorporated into acceptance criteria?
