Health Passports

Small, passport-size booklets are available at ask.hrsa.gov to provide individuals with an opportunity to keep track of their medical history and to learn about organ donation.

The booklets include pages for medical records, appointments, medications, contact information for physicians and other medical personnel, and lists of standard medical tests. Also included are tips on how to stay healthy and information on signing up to be an organ and tissue donor.

There are four versions of the Passports:
