TRANSCRIPT

Hi, I’m Ann Junkins with an important message about your Form 5558 extension request.

The Form 5558, Application for Extension of Time to File Certain Employee Plan Returns, is used to request an extension of time to file Form 5500 series or Form 8955-SSA returns.

If an employer maintains more than one single employer plan and the plan years end on the same date, a single Form 5558 can be used to apply for extensions for up to three of these plans. Please do not include more than three plans on a single Form 5558.

Please do not attach lists of plans to your extension request. Recently, filers have begun attaching lists of plans to the extension form that include several hundred plans.

These lists will not be processed.

Lists attached to the 5558 that are received through July 31, 2012, will be returned to the filer. You will be asked to submit a properly completed Form 5558 with no more than three plans per form for each plan that requires an extension.

Lists received after July 31, 2012 will not be returned and will not be processed.

If you have questions related to the Form 5558, please review the form instructions, visit the Form 5500 corner at www.irs.gov/retirement, or contact the IRS Help Line at 1-877-829-5500.

Thank you.
