Union Positiva has been providing HIV education and prevention services to Miami’s Hispanic population for 10 years, including HIV testing, as well as the implementation of CDC-EBI project. For the last three years they have been providing technical assistance and capacity development for minority HIV/AIDS serving organizations in Miami-Dade and neighboring counties.

Abstract

The purpose of the project is to form a continuum of care for Latinos that are living with HIV and AIDS, or at high-risk for HIV, and teach other minority and Latino serving-organizations how to establish service networks for HIV positives and high-risk Latinos. The project will mainly focus on the large percentage of Hispanics that live with HIV/AIDS in Miami-Dade County. The county is the epicenter of the HIV/AIDS epidemic in Florida with 78 percent of its HIV/AIDS cases, and Hispanics are disproportionately affected by the disease. In addition, high numbers of Latinos do not know their serostatus, and there are low numbers of culturally relevant HIV/AIDS service providers. The project will form a continuum of care for Latino clients that are living with HIV/AIDS and teach other minority and Latino-serving organizations how to establish service networks for HIV+ and high-risk Latinos through:

• Modeling this continuum as a demonstration,
• Providing training,
• Delivering on-site technical ssistance as needed,
• Serving as mentors to smaller, developing organizations and
• Facilitating and participating in community forums delivered.

• Increased number of HIV positive individuals linked with substance abuse, mental health and primary care providers,
• Decreased geographic barriers to obtaining care,
• Improved management capacity,
• Improved Internal Systems,
• Increased number of Latinos tested for HIV and
• Improved cultural/linguistic capacity.

• Number of individuals participating in OMH-funded, grant program activities per year,
• Number and percent of individuals with increased awareness and knowledge of racial/ethnic minority health problems and how to address such problems and
• Number of partnerships facilitated and or/established to enhance coordination of racial/ethnic minority health disparities.

In order to collect, analyze and use the data, the project will use the Performance Evaluation and Monitoring System (PEMS) from the CDC.

• Increased awareness, education, & outreach to address racial/ethnic minority health & health disparities problems
• Improved access to, and appropriate utilization of, health & other community-based services and systems through user-centered design for racial/ethnic minorities (e.g., health IT, culturally/ linguistically appropriate services, service provider education/ training, workforce diversity)
• Strengthened leadership and coordination to leverage resources and enhance effectiveness and efficiency of individual and collective efforts (including, but not limited to, research and data)
