Abstract

The project is an addendum to the existing Collaborative for HIV Services Integration (in existence since 2002), and as part of a continuing effort to improve the health of the targeted populations with HIV/AIDS in South Texas, the collaborative wants to implement the LAAPST project. The purpose of the project is to develop and improve the coordination and continuum of comprehensive HIV prevention, treatment and supportive services provided by 24 organizations in South Texas that are closely interfaced with targeted minority populations impacted by HIV/AIDS. Due to the disproportionate amount of ethnic/racial minority populations dealing with HIV/AIDS in the target region, the project will target these populations. The primary focus will be on Hispanics living with HIV/AIDS. A few of the strategies, practices or interventions that may be implemented include:

• Conducting needs assessments for the agency particiapants,
• Providing technical ssistance to the participants,
• Developing a Technical Assistance Plan,
• Help identifying collaborative partners to link the participants to other social service providers and medical providers serving Latinos living with HIV/AIDS,
• Conducting monthly meetings for education workshops and
• Providing administrative assistance.

• Increase number of, and collaborations between, minority CBOs serving Latinos with HIV/AIDS,
• Establish continuum of mental health and substance abuse services for 24 CBOs and public health clinics serving the target populations,
• Increase sustainability of target CBOs as evidence by systems change thriough fiscal, data management, and program operational practices and
• Increase the number of collaborations and successful grant submissions by improving skill sets regarding grants research and grant writing.

Performance indicators include, but are not limited to, the number of individuals participating in OMH-funded, grant-program activites per year, the number of racial/ethnic health improvement and/or health disparities related to strategic plans and the number of partnerships faciliated and/or established to enhance collaobration.

An outside evaluator will utilize qualitiative evaluation methods of information collection which will include personal and group interviews, telephone interviews, on-site observations, analysis of attendance records, use of individual and group activity logs and comparisons of agency planning documents.

• Increased awareness, education, & outreach to address racial/ethnic minority health & health disparities problems
• Improved access to, and appropriate utilization of, health & other community-based services and systems through user-centered design for racial/ethnic minorities (e.g., health IT, culturally/ linguistically appropriate services, service provider education/ training, workforce diversity)
• Strengthened leadership and coordination to leverage resources and enhance effectiveness and efficiency of individual and collective efforts (including, but not limited to, research and data)
