CDC Online Resource Directory Registration Form
The CDC Online Resource Directory (CORD) is a tool designed to increase communication between CDC and state, tribal, local, and territorial (STLT) public health agency staff. The goal is for CORD users to receive quick responses from CDC staff and contractors regarding public health issues. Users can search CDC employee and contractor contact information and access a phone number or send written communication through the system.
CORD is made available only to STLT public health employees who have registered for access.
