Government Visa Travel

This is a listing of visa requirements for official U.S. passports for entry into foreign countries. The listing shows the number of visa application forms with original signature and photos required to apply for the Visa as well as any other requirements (i.e. letter of invitation, OMEGA printed itinerary, etc.). Please contact the CDC Passport/Visa Services Office (404-718-8890 or email CDCPASS@cdc.gov) for assistance in completing your application package. This statement is located in the first paragraph near the end. The Request for Passport/Visa Services (CDC form 0.1308) is required for all passport and visa requests.

It is important to note that entry requirements for U.S. citizens traveling on official U.S. passports often vary from entry requirements for U.S. tourists. Entry requirements for foreign passport holders also vary and entry requirements must be verified with the CDC Center for Global Health (CGH) on a trip-by-trip basis.

NOTE: It is the traveler's responsibility to obtain letters of invitation, visa receipt numbers (Russia) and visa authorization issuance numbers (Vietnam).

Many visa applications are available online in Adobe Acrobat Reader (PDF) format. You will need the Adobe Acrobat Reader on your computer to open a form. Click on a country name below to open a form or right-click and select "Save Target As..." to save a copy on your computer. Most of these forms have been designed to allow you to fill them in online. You can then print or save a copy to your computer.

Passport size (2x2) photo required. *PLEASE NOTE THAT ONCE THE PASSPORT AND APPLICATION HAVE BEEN SUBMITTED TO THE EMBASSY, THE PASSPORT CANNOT BE PULLED FROM THE EMBASSY UNTIL THE VISA HAS BEEN ISSUED. Passport must be signed. Please allow 7 days for processing. VISA PHOTO MUST NOT HAVE TRAVELER SMILING OR WEARING GLASSES.

Passports must contain at least two clean unstamped pages whenever you enter South Africa. Passport must be valid for 30 days beyond planned stay. Foreign passport holders must include their flight itinerary and hotel confirmation, and letter of invitation from the inviting organization.

Sri Lanka	1	2	Must obtain a visa approval number from the Ministry of Foreign Affairs to submit with the visa application; this is the responsibility of the traveler.
North Sudan	1	2	Visa is required. Passport cannot contain an Israeli visa/entry/exit stamp to enter country. Please allow 6 weeks for processing. Copy of passport. Photos must be 2x2 in size. US tourist and foreign passport holders must include a money order made out to the Embassy of Sudan in the amount of $151.00 If passport has less than six month validity, it must be renewed prior to submitting it for a visa.
South Sudan	1	2	Visa is required. Visas can no longer be obtained upon arrival; must obtain prior to departure. Passport cannot contain an Israeli visa/entry/exit stamp to enter country. Copy of passport. Photos must be 2x2 in size. If passport has less than six months validity, it must be renewed prior to submitting it for a travel permit. Normal processing time for obtaining a Government of Southern Sudan travel permit is @ 7 business days from time application package is submitted; please plan accordingly.

Vietnam	2	2	Visa authorization issuance number (VAIN) from Hanoi Office of Immigration is MANDATORY (traveler's responsibility to obtain). In-country POC can assist in obtaining the VAIN.
