Program Success Stories


Funded communities in CDC's Healthy Communities Program are working to reduce the burden of chronic diseases and achieve health equity by implementing population-based strategies that create sustainable systems and environments. Communities apply evidence-based strategies to improve access to nutritious foods, increase physical activity opportunities, and reduce the initiation and consumption of tobacco use as well as exposure to secondhand smoke.
