REACH

CDC's Racial and Ethnic Approaches to Community Health program (REACH) serves as the cornerstone of CDC's efforts to eliminate racial and ethnic health disparities among African Americans, American Indians, Hispanics/Latinos, Asian Americans, Alaska Natives, and Pacific Islanders. REACH grantees use community-based participatory approaches that address social determinants of health through environmental change strategies. Communities implement and evaluate innovative strategies for improving health in communities, health care settings, schools, and work sites.

REACH funds 50 communities and six minority-serving national organizations. Nine state Racial and Ethnic Health Disparities Action Institute (REHDAI) teams were chosen to build health equity at the local and state levels through collaboration with other REACH communities.
