This primary purpose of this site is two-fold:
  1. Enable the ability to electronically submit Service Difficulty and Malfuction/Defect reports.
  2. Allow the public to search/review all the submitted reports.

Q. How do I submit a supplmental record?
A. To do a supplemental without having to retype everything, simply fill out the new record using the same control number of the original record. We will see both records here and update the initial record with the new information. Please place the words "supplemental information" in the problem description field to alleviate any confusion. If you need any assistance, please email or call us.

Q. How do I submit photographs relating to a SDR?
A. If you have photographs that you feel better shows the SDR that you are submitting, please send an email to 9-AMC-SDR-ProgMgr@faa.gov with the photograph attached along with the Record ID number (Air Carrier) or date of submission and aircraft ID (General Aviation).

What's New In SDR?


August 24, 2009

A page where registered users can view their recently submitted SDRs has been added. The status of each of your submission will be listed and the entire report can be viewed/printed.

Also added is the ability to wildcard the JASC code field on the query screen. For example, if you want to see SDRs related to ATA code 27 you would type in '27%' and records with JASC codes 2700 to 2799 would be returned.

The final additon for this release is a modification to the login system which will allow users with accounts created before July 6th, 2009 to login with either their email address or their chosen user name; after that date all newly created accounts exclusivly use the registered email address as the user name.
