CDC Community Transformation Grants

The Community Transformation Grants program will support
community efforts to reduce chronic diseases such as
heart disease, cancer, stroke, and diabetes. By
promoting healthy lifestyles, especially among

disease, these grants will help improve health, reduce

National Diabetes Education Program

The National Diabetes Education Program is a partnership
between CDC and the National Institutes of Health. This
Web site provides tools and publications in a range of
languages for people who have diabetes, those who care
for people with diabetes, and those at risk for the
disease, as well as for public health practitioners,
community health workers, and health care professionals.

Native Diabetes Wellness Program

The Native Diabetes Wellness Program helps American
Indian and Alaska Native communities develop effective
strategies for diabetes care and prevention. The Web
site features the Eagle Books, a series of children’s
books for Native American children and others interested

type 2 diabetes, such as by being more physically active
and eating healthy foods.

Vision Health Initiative

state. States that used the Behavioral Risk Factor
Surveillance System’s vision module can produce reports
