CDC’s Division of Diabetes Translation (DDT) announces:
The National Program to Eliminate Diabetes Related Disparities in Vulnerable Populations
This five-year program has been developed to reduce morbidity and premature mortality, and to eliminate health disparities associated with diabetes in many communities and regions across the country.
How will the program achieve these goals? The program will fund six organizations to mobilize, empower and assist community partners in planning, developing, implementing, and evaluating community-based interventions to reduce the risk factors that influence health disparities associated with diabetes.
How will this program enhance existing efforts?
The program will complement and strengthen the reach and
impact of the Diabetes
Prevention and Control Programs (DPCPs), the
Native Diabetes Wellness
Program (NDWP), and the
National Diabetes Education Program (NDEP).
