Information Notice 2003-02, Recent Experience With Reactor Coolant System
Leakage and Boric Acid Corrosion dated January 16, 2003
The U.S. Nuclear Regulatory Commission (NRC) is issuing this information
notice to inform addressees of recently observed reactor coolant leakage
at two pressurized water reactor facilities, one of which resulted in the
subsequent degradation of the reactor pressure vessel head.

Subject: SURRY UNIT 1 C REACTOR COOLANT PUMP MOTOR BEARING FAILURE

Discussion:
On 1/14/03 Surry Unit 1 was manually tripped from full power due to
elevated C reactor coolant pump (RCP) shaft vibrations and a rapidly
increasing lower motor bearing temperature. The lower motor bearings
revealed severe damage due to lack of lubrication. Investigation
revealed that the lower bearing oil reservoir site glass was empty and
the reservoir was approximately 4 gallons below the 22 gallons required
to fill the site glass to mid-scale. The investigation also revealed
that no oil low level alarm had been received due to a broken oil
reservoir level switch assembly. Furthermore no adverse trend in the
bearing RTD temperature had been noted. The RTD was not installed in
accordance with the Westinghouse drawing, in that, the RTD tip was not in
contact with the bearing shoe. The loss of oil apparently occurred
through small leaks over the approximately 400 days the unit was in
service.
The licensee has inspected oil levels and functionally tested the level
alarms for the other Unit 1 RCPs. The B RCP upper oil reservoir alarm
failed and was repaired. The C RCP motor bearing has been repaired and
the unit is preparing for restart.
